TENANT SERVICES

ATTENTION!!!! 
If you reside in the city of Omaha and have a COVID-related issues, be sure to indicate it on your application as you may qualify for COVID Relief Assistance. You must provide proof of COVID hardship, and your income must meet the guidelines. If approved, you may be eligible for up to three (3) months of rental assistance and one (1) month of utility assistance. One of our Housing Specialists will provide you with additional details upon review of your application, as additional information will be required to determine eligibility. Processing may take up to 10 business days if all documents are submitted and received promptly. 
  
Note: If you request utility assistance under COVID hardship, we cannot pay future months of utility assistance. If approved, we can only pay for past-due utilities, including the current month. 
  
Step 1: Click on this link to complete the Intake Application -  https://form.jotform.com/210386060996057 
Step 2: Upload the following documents on the Authorization and Liability page of the application. Here are the documents you’ll need to attach:   
  
·        ID
·        Social Security OR ITIN OR IRS Tax document  
·        Lease (for Rental request)  ALL PAGES
·        Utility bills (must be in the name of the applicant)
·        Landlord contact information (for rental requests)   Name, email address and phone number
·        Notice (e.g. 3, 7, or 14-day notice) OR a ledger OR a statement from the landlord of the balance owed. You may also upload a court summons if you have one. 
·        Proof of hardship to show why you fell behind i.e. letter from employer, paycheck stub, bank statements, medical statement, Proof of COVID. 
·        Proof of Income from the last 60 days including but not limited to: Paystubs, Unemployment comp, Child Support, *Bank Statements, Social Security, SSDI…  If no income please complete the Zero income form in Step 6.
 
* If you have any bank accounts, you must provide bank statements for ALL accounts you have. Please provide complete bank statement to include the last page even if the last page is blank. 
 
Step 3: Please be sure to watch your email to see if additional documents are needed to process your application. If additional documents are requested, you can attach them to the email (you will not need to submit a new application). Processing can take up to 15 business day to determine eligibility

If you had no source of income in the last 60 days, please complete step 6 (Zero Income form) below along with your full bank statement from that last 60 days.
